Overview

Terros Health is pleased to share an exciting and challenging opportunity for a Recovery Navigator based out of our 51st Ave Healthcare Center in Phoenix, AZ. You will be responsible for outreach, coordination of care and assessment of clinical needs in order to inspire a change for life in the patients paneled to Terros Health as their Behavioral Health Home. The individual will need to be professional, friendly, a self-starter, organized, and compassionate.

Responsibilities

Terros Health is hiring a Recovery Navigator at our 51st Ave Healthcare Clinics in Phoenix, AZ. 

 

Full-Time, Employed

Location: 51st Ave and Camelback in Phoenix, AZ

The Recovery Navigator will work with individuals diagnosed with a Serious Mental Illness by conducting outreach by phone and in the field to engage and encourage clients to participate in treatment opportunities including psychiatric, case management, peer support, and/or primary care services.

 

Duties Include:

  • Completes Navigator assessments, crisis plans, treatment plans, and health risk assessments, and coordinates or refers for services identified.
  • Collaborates with assigned Clinical Director to review assessments for determine level of care needed for clients.
  • Embraces the recovery philosophy and promotes a safe recovery environment for clients.
  • Uses advanced Motivational Interviewing strategies to engage and support clients receiving services.
  • Works under the direct supervision of the Navigator Program Supervisor.

Qualifications

  • The applicant must have the minimum combination of education and/or experience:
    • A bachelor’s degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
    • A bachelor’s degree in the field of behavioral health and any behavioral health experience, or
    • A master’s degree or higher in the field of behavioral health.
  • Must have valid Arizona driver’s license, be 22 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy
  • Must have a valid Level 1 Arizona Fingerprint Clearance card or apply for one within 7 working days of employment.

  • Certified Case Manager preferred
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
  • Excellent communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
  • Advanced Motivational Interviewing and engagement skills.
  • Basic understanding of the DSM-V preferred.
  • Computer skills:  Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
