You will be responsible for outreach, coordination of care and assessment of clinical needs in order to inspire a change for life in the patients paneled to Terros Health as their Behavioral Health Home. This role will work with individuals diagnosed with a Serious Mental Illness by conducting outreach by phone and in the field to engage and encourage clients to participate in treatment opportunities including psychiatric, case management, peer support, and/or primary care services. Completes Navigator assessments, crisis plans, treatment plans, and health risk assessments, and coordinates or refers for services identified. Collaborates with assigned Clinical Director to review assessments for determine level of care needed for clients. Embraces the recovery philosophy and promotes a safe recovery environment for clients. Uses advanced Motivational Interviewing strategies to engage and support clients receiving services. Works under the direct supervision of the assigned Clinical Director.
Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 20 working days of employment.
