    Responsibilities

    Under the Agency’s policies and professional requirements: Provides direct care services (personal assistance and ILS skills training) and supervision of clients of permanent supportive housing and behavioral health services; Coordinates client referrals, case management services as needed, tasks involved in serving the clients of the Community Living/Permanent Supportive housing services; Shall fully comply with all Permanent Supportive Housing service requirements per SAMHSA evidence based practice guidelines. Works with other Program staff, Program Supervisor, Director and medical records staff;  provides health and wellness and recovery oriented services; Participates in program evaluation procedures and professional record keeping;  Works under the direct supervision of the Program Supervisor.

    • Provides direct face to face community based services to assist tenants to maintain their housing. Providing PSH services in both scattered-site and single-site (YTV) sites assisting tenants to maintain their housing. If 24 hour 1-1 service, keeps clients in line of sight at all times.
    • Provides outreach for the purpose of engaging tenants into PSH services. Orients potential tenants to the PSH program.
    • Consistently meets Service Delivery expectations, which is no less than 62.5% of paid time.
    • Provides transportation to tenants as appropriate and as medically necessary, and determined by the PSH team, in personal vehicle or in Terros Health provided vehicle, when available.
    • Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate. Coordinates care with a tenant’s family, behavioral and general medical providers, community resources, and other involved supports.
    • Assists tenants with connection to physical health services, including Primary Care services and medical appointment follow up.
    • Provides direct care wellness management and recovery strategies using strength-based engagement and psycho-social/educational /rehabilitation services/groups as needed. Ensures linkage into community services and resources.
    • Maintains appropriate knowledge, skill, and trainings for this position. Completes all required e-learnings and live trainings on time.
    • Actively engages tenants into services by utilizing motivational interviewing techniques creating an alliance that will support their success in reaching their treatment plan goals.
    • Employs therapeutic interventions for life skill development, participating in meaningful activities and daily living skills appropriate for identified goals on treatment plan including assertive outreach and engagement strategies.
    • Ensures the development and implementation of transition, discharge and aftercare plans prior to discontinuation of supportive PSH and behavioral health services.
    • Seeks appropriate clinical supervision to ensure the provision of effective services.
    • Maintains knowledge of procedures for other Recovery Services teams and provides direct services for other team clients when capacity warrants.

    Qualifications

    • High School Diploma or GED, as well as a combination of behavioral health education and work experience totaling a minimum of three (3) years; or an Associate’s degree in any field, plus two years of work experience in a behavioral health service delivery working with Serious Mental Illness.
    • Customer service skills, especially good communication skills are needed.
    • Ability to work independently, with flexibility, and within prescribed deadlines.
    • Basic computer skills.
    • Valid AZ driver’s license and proof of insurance. Driving is an essential function of this position.
    • Level 1 fingerprint clearance.
