    Terros Health is pleased to share an exciting and challenging opportunity for a Community Health Worker, working at our East McDowell Road location This individual will provide ongoing services as Community Health Worker to patients in need of care management services for the purpose of establishing and reaching appropriate health-related goals.The individual will need to be professional, friendly, a self-starter, organized, and compassionate.

    This position is responsible for both clinical and administrative tasks, including outreach and engagement efforts with patients referred for specialty and patients needing group or one to one health related services in the community. Interacts with patients and works a part of the care team. Will be knowledgeable related to population health management and engaging vulnerable populations.  Will perform program related activities as assigned, facilitate teamwork within the organization, and work under the direct supervision of the Site Administrator.

    • Meets patients in the community to provide health literacy information and resources to have their health care needs met.
    • Follows up on the patient related phone calls and coordinates a response back to patients based on consult with the Care Management Team.
    • Develops and maintains positive community-based relationships with healthcare partners to benefit patient care coordination.
    • Assists in coordinating transportation, driving patients, and helping patients keep appointments.  Also works to understand the purpose of patient visits and the follow-up needed.
    • Assists patients in transitioning back to the home environment following hospitalizations and ensures they have medications, access to adequate food and supplies in their homes in a timely manner.
    • Follows up on missed medical appointments to appropriately reengage the patients.

    Ability to drive to meet patients at least 50% of the time.



    • Must possess: 
    • Excellent communication skills.
    • Community Health Worker certification not required, but highly desirable.
    • Peer in behavioral health or medical area.
    • Excellent interpersonal skills both in person and by phone, with high professionalism.
    • Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience and meet requirements of Terros Health’s driving policy.

    • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 20 working days of assuming role.
