    Terros Health is pleased to share an exciting and challenging opportunity for a Care Coordinator, working at our McDowell RD location This position provides and coordinates supportive services for the integrated patient population utilizing population health tools to engage the care team and patient. The individual will need to be professional, friendly, a self-starter, organized, and compassionate.

    Responsibilities

    This position contacts and coordinates patients related to population health preventative measures needing to be scheduled.  Works with the patient to create care plans to address individual patient needs that require care management.  Interacts with patients and works as part of the care team. 

    • Promote timely access to appropriate care by serving as the contact point, advocate and informational resource for patients, family/caregivers, payers and community resources.
    • On a daily basis, facilitates patient access to appropriate medical and specialty providers utilizing patient registries and gaps in care reports.
    • Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction and reducing health care costs.
    • Develops short-term and long-term interventions in the development of expected patient outcomes; collects data through patient tracking in order to measure outcomes
    • Assist with the identification of “high-risk” patients and add these to the patient registry or flag in EHR.
    • Increase utilization of preventative care; reduce emergency room utilization and hospital readmissions; promotes and reinforces patient centered medical home concepts.
    • Increase patients’ ability for self-management and shared decision-making.
    • Provides outreach and engagement activities in person and telephonically.

    Ability to travel to company sites throughout the state of Arizona.

     

     

     

    Qualifications

    • Education/Experience:
      • GED/High school diploma with a minimum of 3 years of experience in clinical setting or
      • Bachelor’s degree in non-behavioral health field plus 1 yr of experience in clinical setting or
      • Bachelor's degree in behavioral health field or  
      • Master’s degree or higher in behavioral health field. 
      • Preferred - Minimum of 1 year of previous MA experience. 
    • Must possess: 
      • Well-developed critical thinking and analytical abilities, and
      • Time management, organizational, conflict resolution and judgement skills
      • Excellent communication skills including writing, proof reading and speaking
      • Proficiency using Microsoft Office suite and EHR.
    • Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience and meet requirements of Terros Health’s driving policy.

    • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 20 working days of assuming role.

    • Knowledgeable related to population health management and engaging vulnerable populations.
