    Seeking a Community Health Worker to join our Care Management Team for a full-time position at our 27th Avenue Integrated Care location.

    Community Health Worker/ Certified Peer Support Specialist needed to join our Care Management Team at our 27th Avenue Integrated Care location in Phoenix, AZ.


    This position is responsible for sharing their perspective to families of adults with physical health and behavioral health needs who are transitioning to the community from the criminal justice system. The goals of transition are to integrate primary care and behavioral health services for the purposes of better coordination of the prevention and chronic illness care. This specialist provides supportive services for the providers and patients family to assist them with reaching their integrated care goals. This position is responsible for both clinical and administrative tasks, including outreach and engagement efforts with patients referred for specialty and patients needing group or one to one health related services in the community.


      • Meets patients and families in the community to provide health literacy information and resources to have their health care needs met.
      • Interacts with patients and works a part of the care team.
      • Attends clinical and operational meetings as indicated to improve processes.
      • Knowledgeable related to population health management and engaging vulnerable populations
      • Encourages families when supporting their loved ones to maintain healthy limits and to support sharing of information for better health outcomes.
      • Ability to blend personal experience into a professional role. Must be able to share common experiences with recipients and family members in order to engage, support and inspire others.

    • Arizona state Peer certification
    • Lived experience assisting a loved one in navigating the behavioral health system and criminal justice system
    • Valid Arizona Driver’s License and proof of insurance. Driving is an essential function of this position.
