This person will function as a Case Manager on the ACT team. Coordinates, monitors and provides direct services in a community setting for clients served on the ACT team. Works as a member of a clinical team to assess client needs. Participates in the development of services plans, provides/coordinates services related to the service plan. Acts as a client advocate, coordinates services and discharge plans, and assists in crisis intervention. Provides on-call crisis intervention services, as well as weekend and holiday coverage.
