    • Assembles patients’ health information in chart.
    • Ensures all forms are accurately completed, properly identified, and signed.
    • Ensures all necessary patient information is in the computer and up to date.
    • Preps and scans pertinent information into 3rd party billing system and EMR.
    • Processes or assists processing medical record requests, subpoenas, and non-routine requests. Forward billing requests for processing.
    • Serves as resource on medical records/phi and assists and educates staff.
    • Monitors onsite chart reviews by 3rd parties and adheres to review process.
    • Identifies and/or reports breaches appropriately.
    • Performs other duties as assigned.


    • High school graduate or equivalent;
    • Strong organizational skills;
    • Experience in behavioral health medical records department setting preferred;
    • Ability to develop reports and spreadsheets in Microsoft Word and Excel;
    • Ability to read and communicate effectively in English, and
    • Additional languages preferred but not required.
