    Primary Care Nurse Practitioner Needed in Phoenix, AZ - 27th Ave Integrated Care & 27th Ave/Indian School


    • Correctional Medicine Experience Preferred
    • Full-Time: 40 hours/week: Monday-Friday
    • 27th Avenue Integrated Care (27th Ave/Indian School) & Future Office located at I-17/ Indian School Rd
    • Excellent Base Salary

    • Patient Population: Underserved and some correctional medicine (patients on probation)


    Direct Services:

    • Physical Exams
    • Immunizations
    • Lab Services
    • HIV/STI Testing
    • On site Pharmacy
    • Same Day and Walk-In Appointments Available


    • Nurse Practitioner AZ License
    • Master’s degree in nursing or DNP in Family Medicine or Adult Medicine
    • Possess a valid First Aid/CPR and AED certification
    • Able to pass a background check and possess a valid Fingerprint Clearance Card
    • Possess a valid DEA license
    • Experience in the specific area applied for
