    Seeking a dynamic Internal Medicine or Family Physician who is interested in working with Terros Health in a growing, innovative field of behavioral health and primary care integration in Phoenix, AZ.


    This is a FULL TIME (40 hours/week) position in the west Phoenix area.

    Schedule:  Monday-Friday, 5 - 8'S

    As Lead Physician for the Primary Care Program; provides leadership relating to the clinical supervision of the primary care medical staff (NPs and PAs).  The Lead PCP is responsible for providing direct patient care approximately 80% of the time; insuring continuous quality improvement in all clinical activities; and acting as an integral part of the clinical integrated health team of the Integrated Health centers.  Some travel is required.


    DUTIES AND RESPONSIBILITIES include the following, but not limited to:


    Oversees Primary Care Medical Services: 

    • Oversees daily clinical practice of the Care Clinic (Primary Care).
    • Values and leadership are in alignment with the Terros vision and standards of care.
    • Provides clinical leadership and oversight within the Triple Aim of Excellent Patient Experience, Improved Quality and Cost.

    Oversees Medical Providers and Staff:

    • Supervises the performance improvement activities of the health center that relate to the clinical program including the clinical care measures.
    • Provides direct supervision to primary care providers in the performance of their duties and responsibilities.
    • Assists Integrated Medical Director in organizing the clinical staff and resources in carrying out the health care program of the health center, including but not limited to, integration of behavioral health and other medical resources in the community.
    • Assists Lead RN in coordinating roles and responsibilities of Medical Assistant and Nursing staff.

    Terros Leadership:

    • Participate in developing protocols for high complexity patients under direction of Integrated Medical Director.
    • Responsible for leading the community integrated health centers’ daily clinical meetings.
    • Assist in collection of quality measures (UDS, PQRS, and VBSL) indicating ongoing health improvements.
    • Active participant in modeling integrated care between primary care and behavioral health.
    • Ensures patients are seen when they present for care.

    Represents Terros:

    • Participate in professional committees and offsite meetings as determined.
    • Participates in Morbidity and Mortality reviews as directed by Integrated Medical Director and/or CMO.
    • Participate in the Arizona Alliance as requested.

    The Organization:

    • When directed by Integrated Medical Director and/or CMO, assists in development, review, and implementation of clinical practice guidelines to assure coordinated, quality patient care.
    • Reviews clinical practices by maintaining systems and participates in medical record audits, medical provider peer reviews, and other practice review activities.
    • Supports the HRSA requirements and outcomes as part of the FQHC lookalike/FQHC.

    Clinical Practice: 

    • Provides direct primary care services to patients.
    • Completes medical records using collaborative documentation by close of each business day.
    • Maintains current certification and meets all continuing education requirements. 
    • Develops differential diagnostic assessments, risk assessments, patient education, and progress towards treatment goals including medical, psychological, social, behavioral and developmental aspects of the patient’s situation. 
    • Documents patient’s progress with delivery service expectation of 62.5% on the clinical record in addition to medication documentation according to established policy. Completes medication education and informed consent. 
    • Orders follow-up blood levels and other lab tests according to standard medical practice, obtains a past and current medical history. Initiates contact with other PCP, ED clinicians, and BHMPs according to established guidelines; practices according to evidence-based practices. 
    • Provides psychopharmacological treatment in alignment with patient’s treatment plan; appropriate follow-up of patient’s symptoms and response to medications, including side effects. Utilizes appropriate medication based on best practices, in addition to appropriately completing required documentation for preauthorization of pharmacy services. 
    • Identifies potential barriers to quality care and opportunities for improvement and reports them to the Integrated Medical Director and/or CMO. 
    • Participates in medical staff meetings including monthly meetings; participates in all required site meetings; and participates in any meeting as requested by Integrated Medical Director and/or CMO. 
    • Participates in training and supervision of graduate medical students and postgraduate residents when available, and completes all necessary supervision evaluations.

    • Education: MD/DO
    • Field(s) of study: Graduate of an American or Canadian medical school accredited by the Accreditation Council for Medical Education (ACME) or equivalent training in a foreign medical school with successful completion of the ECFMG and FLEX examinations. Full training in a residency program in the United States or Canada that is approved by the ACGME.
    • Experience: Ideally 3+ years after completion of Residency training, with some supervisory experience.
    • Industry:  Job Specific: Post-residency experience of at least 3 years involving substantial direct patient care during this period at multiple levels of care. Clinical experience pertinent to the patient population(s) being managed.

    Other: Aware of current research and use of evidence based practices in treatment.


    Computer Skills: Proficient in Microsoft Office (Outlook, Excel, Word, PowerPoint), and proficient in Electronic Medical Records “EMR”


    Licenses, Certifications, etc.: Must have current active license to practice medicine in the U.S.; licensed or licensure eligibility in state of operation; licensure must be complete within the 1st year of employment; Active DEA.  Board Certification in specialty.


    Travel: Local and state travel required.  Out of state travel may be required.
