Responsibilities

Under the Agency’s policies and professional requirements, with oversight from medical/clinical staff, provides supportive services for the medical/clinical practitioners and medical/clinical clients. Responsible for follow-ups, support and education of the medical/clinical clients. Provides coordination with pharmacies, RBHA, and other medical/clinical providers, as well as completing Prior Authorizations. The Care Navigator will perform program related activities as assigned, facilitates teamwork within the organization and participates in evaluation procedures and professional record keeping, works under the direct supervision of the T3 Program Coordinator.

Qualifications

  • High School Diploma/G.E.D and at least two years in HIV prevention or related field
  • Associates or Bachelors degree preferred.
  • Spanish speaking preferred, but not required
  • Experience in the specific area of work applied for
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 20 working days of employment
