Responsibilities

This position provides support to members, families and teams of those individuals who are part of the Developmentally Delayed population including the Autism Spectrum, children age birth to five with behavioral needs, and transition age youth (TAY). The program works both with children and adults. The purpose is to provide behavioral support and consultation to the individual member, their family and/or setting staff. Responsible for responding to crisis calls as needed as well as planned interventions. Capable of applying deescalating   interventions based on positive behavior support, choice education, and person and family support. Will assist with the development of individual service plan, person centered planning, emergency contact sheet, and the at risk crisis plan. Will participate in emergency staffing and CFTs.   On call position: will cover minimum of four 10- hour shifts per month.

  • To work with other team members in providing support and knowledge as needed on shared cases.
  • Appreciates and applies best’s practices, recovery approaches and person support interventions.
  • Performs crisis de-escalation; mental status and risk assessment; safety planning and positive behavior support based on clinical expertise and the involvement of the client and their families and caregivers.
  • Contribute to a culture of learning (e.g., sharing information, discussing articles and best practices, etc.).
  • Complete ongoing continuing education per company policy.
  • Works in collaboration with individual members’ team and other stakeholders.
  • Attends CFTs and other client centered staff meetings as needed.
  • Upon receipt of referral reaches out to individual and team within 24 hours.
  • Completes required documentation in electronic medical record by midnight of date of service.
  • Provides timely coordination of care with client’s teams in a professional manner(phone contacts, emails, staffing, meetings, written reports/notes, and conversations)
  • Cover minimal of four shifts per month (on call staff)
  • Meet with placement and child to create a treatment plan within 7 days of initial contact.
  • Provides directs support, positive behavior support and/or behavioral consultation to clients, family and/or teams.
  • Make reports and file incident reports based on Duty to Warn, Dutay to Report or other significant emergency situations within established timeframes according to Terros Health Policy.
  • Completes all monthly reports by the 5th of the month, reporting progress, outcomes and ongoing goals and sends to referring agency as needed.
  • Ensures that all legal documentation is signed and scanned into child or adolescent chart within 48 hours, maintaining all documentation in an agency designated case file.
  • Maintains records of mileage traveled in course of job performance and submits to program director bi-monthly.
  • Regularly attends scheduled program staff meetings, weekly supervision and agency required events.
  • Staff is willing to work nights and weekends if it meets the needs of their clients, coordinating their schedule with the client’s families, HNCM and supervisor.
  • Update Outlook Calendar to reflect client appointments and scheduling of Child and Family Team Meetings, School Appointments and work dates and times.
  • will use a strength-based coaching approach that educates and models interactions by offering environmental evaluation and recommendations to connect children and families with unexplored supports, increased coping skills, reduction in maladaptive behaviors and increased medication compliance
  • Staff will participate in Child and Family Team - CFT meetings, interface with Division of Developmental Disabilities - DDD, therapists, counselors and Child Protective Service - CPS as part of their job requirements, supporting healthy and positive outcomes

  • Provides Functional Behavioral Assessments to members as needed.
  • Performing other duties necessary to the proper functioning of the agency.

Qualifications

Bachelor’s Degree or 2 to 3 years’ experience working with children in a Behavioral Health Setting.

  • The person must be able to work well and tactfully with individuals and groups.
  • They must possess a valid Arizona’s Driver’s license, and be able to properly and safely operate a Terros Health vehicle. Incurred traffic violations may be subject to disciplinary action.
  • The person must have the ability to perform first aid, and cardiopulmonary resuscitation as necessary and appropriate. The person must also have a current certification in CIT.
  • Confidentiality of client records shall be maintained during and after employment at Terros Health. Failure to do so may result in dismissal.
  • Physical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job.
